Given how you get arrested, I'd have to suggest hardcore harm reduction as the first step. I mean no judgment there; I have a lengthy alcohol-related criminal history of my own.
Are you dealing with any consequences right now? Whether yes or no, do you have a plan in place to avoid repeating the mistakes that ended in arrest? Everything's just going to keep getting worse and worse if you can't stay out of the court system.
There's not that much to the Sinclair method. If you're taking naltrexone one hour before drinking alcohol, you've got the gist of it down pat. After that, it's mostly a matter of time for the cure to work.
You never really got a chance to try TSM, because of the previous arrest. IIRC, you felt early effects like increased awareness. Ideally there'd be a drop in consumption by week 10, but not always, so that doesn't mean you weren't responding.
You know, I hate to say it, but I don't see any way around it. You're in the same place you were a year ago, only worse.

You're facing essentially the same choice that you were then. TSM is unique in that it may have a real cure for you, but it does take time, so in your case it really, really needs to be combined with harm reduction. I'd really hate to open this forum in a few weeks to learn that Neptune is going white-knuckle abstinent because of another arrest.
