 Post subject: some questions about TSM/nal

Well it turns out I have an appointment with DASH (drug and alcohol service in my home town) in seven weeks. They prescribe naltrexone, so I will be on it soon - I will also be able to access a counseling service. I get the golden rule, one hour before drinking pop a nal and then go for it. I just have a few questions I won't be able to ask DASH as they do not follow TSM.

1. How do you prevent alcohol poisoning? I'm worried that by taking nal and not getting that 'high' I might just drink more and more chasing that buzz.

2. Does anyone know of any app that allows you to keep a drinking diary for an android phone?

For what it's worth, I've never heard of anyone that this has happened to. I suppose you could ask a friend to hang out with you or check in with you, if you're really worried about it.

On the starter dose of 25mg, you'll probably still be feeling some of the effects of alcohol. Most of us do. That being the case, you may not even notice much difference. Based on our collective experience here, most of us don't find the first time to be all that dramatic.

It's up to you, really. If your standard levels aren't that high and you're truly worried about a big uptick in chasing the buzz, you might feel better starting at an even lower dose (12.5mg?). That would create so little blockage that you would almost certainly not notice it, so your first couple of times would have no practical difference from what you've already been doing. You could also keep in touch with someone via text or online chat, if nobody's available to be with you physically.

I just used a pen. I made a mark on my hand each time I opened a new beer. I wrote it into the diary (a text file) the next day, although after the first few times I was able to finish the diary on the same night.

Let us know how it goes, okay? We love a good success story, and we're pretty good at helping with problems, too.

seeker...good luck to you. Just keep in mind that Naltrexone doesn't technically block the overall effects of alcohol, just the opioid receptor part of it. So, alcohol poisoning would be extremely hard because you'd still be drunk and tired, with loss of coordination and slurred speech and all, just (possibly) without the "crazy" feeling. I imagine nearly everyone would just give up and go to bed at some point, and give up "chasing the high" (I never really "chased" the high until I was already high, if you know what I mean). The way I experienced it was that Naltrexone heightened the negatives of drinking (making me more uncoordinated than I would have been, or more fuzzy-headed) and decreased the positives (no good euphoria).

ALSO, keep in mind (very important) that for many / most people, TSM works at the subconscious level. It does its thing without you actually being conscious of it. You do it over and over and over and over, and then one day, you realize you don't need to drink anymore, or you have a level of "will power" you didn't have before. It's subtle, yet strong.

I don't specifically remember this, but I would not be at all surprised if someone had had that response. People can be determined to get their buzz on. (But it also would not surprise me if someone who did this were to stop complying with TSM protocols in general, because it sounds to me like such a person is making a deliberate choice to drink past what their craving demands.)

But I'm pretty certain that no one should worry about alcohol poisoning resulting from TSM if they aren't worried about it in the course of day-to-day drinking.

I think what is being referred to is drinking past the regulatory effect of nal. Yes this happened to me during TSM however I still never drank the amount I did pre-tsm which was rather a lot anyway!

Sometimes drinking levels can spike or get even worse than pre-tsm drinking, this is part of the extinction process however if you are mindful I think much of this can be prevented.

My experience and from what I have seen posted here and elsewhere is that TSM requires motivation and co-operation to be successful.

That's true of anything, isn't it? I can't think of any method which could be forced on someone against his will. Extreme aversion therapy, maybe, but even that wouldn't last after the first few times the person pushed through the induced aversion.
